About ONX EjectoLift NX | Animated FiveM EMS Gurney Vehicle
This FiveM EMS gurney vehicle, the ONX Brute EjectoLift NX, transforms patient handling on your server from a stack of prop hacks into a clean, scriptable medical workflow. Built as a true vehicle entity rather than an attached prop, this scratch-made gurney is meant to be driven by medics, loaded into ambulances, and used for genuine patient transfers with proper seat nodes and animated height control.
For roleplay servers where EMS calls matter — from a single highway crash to a full hospital handoff inside Pillbox — the difference between a real stretcher and a faked one is enormous. Players see the handle drop. They see the gurney rise as it locks into the ambulance. They stay on the bed during transport instead of teleporting between scenes. That kind of immersion is exactly what the EjectoLift NX is engineered to deliver.
Scratch made by Tidemo with contributions from WiseGuy, and designed to pair with the ONX ambulance fleet of six models and their loader configurations: empty loader, raised hooks, gurney stretcher, and gurney stretcher with raised back rest.
🏥 Why a FiveM EMS Gurney Vehicle Changes Roleplay
Most stretcher solutions on FiveM rely on attached props, frozen entities, or workarounds that break the moment a patient ragdolls or a script reloads. Treating the gurney as a vehicle entity flips the problem on its head: it has its own physics, its own seats, its own animations, and it behaves predictably for every framework you throw at it. EMS workers can take the driver seat to push and steer it, while patients drop into the passenger seat and stay locked to the bed without grabbing vehicle control. The result is patient transport that finally functions like a proper FiveM ambulance stretcher mod instead of a clumsy prop puzzle.
✨ Core Features & Animated Behavior
Every feature on this gurney is built to support believable patient handling outcomes, from the first scene response to the hospital intake:
- 🛞 Gurney built as a vehicle entity for easy control and clean scripting across QB, ESX, and custom frameworks
- 🧍 Driver and passenger seat nodes — medics take the driver seat to control the gurney, patients sit in the passenger seat to stay on the bed without taking control
- 🛏️ Two gurney variants: flat and raised back rest, so you can match the scene (transport vs treatment)
- 🎨 Colorable frame paint to match your service or hospital branding
- ⬆️⬇️ Animated height adjustment — hold H to raise and lower the gurney
- 🪝 Handle behavior built in — the handle raises automatically in the lowered position for visual accuracy
🫴 What's Included in the Package
- ✅ ONX Gurney Vehicle
Driver and passenger seat nodes
- Flat and raised back rest options
- Colorable frame paint
- Animated raise/lower via H
**
Note: This package provides the asset**. The exact push, load, and transfer logic is intentionally left script-open so you can implement it for QB, ESX, or custom frameworks without fighting prebuilt behavior.
✅ Integration Options With ONX Ambulances
The EjectoLift NX is designed to slot into the ONX ambulance fleet through two clean integration paths that your developers can wire up however they like.
1) Load into ONX ambulances (bone attach)
Script the gurney to attach to the misc_s bone on any of our custom ONX ambulances, so it locks into the loader visually and functionally.
2) Transfer patient inside the ambulance (seat swap)
- Switch the patient from the gurney to a seated position inside the ambulance during transport
- Pair the swap with the ambulance's matching gurney mod part so the visual position and animation line up with the gurney vehicle seating
📦 Quick Installation
- Add the ONX vehicle resources to your server's resources folder inside a folder called [onx_vehicles].
- Add ensure [onx_vehicles] to your server cfg.
- Configure your ambulance and gurney behavior using your preferred framework or custom scripts.
🛠️ Developer & Technical Specifications
Performance is tight across the board, with full LOD coverage so the gurney holds up at any draw distance on a busy multi-unit EMS scene.

🧩 Compatibility & Framework Notes
This FiveM EMS gurney vehicle is framework-agnostic by design. Because the push, load, and patient-transfer logic is left intentionally script-open, your developers can wire it into QBCore, ESX, Qbox, or any custom EMS framework without fighting hardcoded behavior. A subscription is optional rather than required — the EjectoLift NX is sold as a standalone asset — and Game Build 3095 is needed for some features. Support is included for licensed subscriber servers with assets granted in your Cfx keymaster portal.
📜 Licensing & Redistribution Terms
The community is explicitly allowed to:
- ✅ Create custom scripts that interact with ONX gurneys and ONX ambulances
- ✅ Sell those scripts commercially
With the following restrictions:
- ❌ Do not redistribute, reupload, bundle, or include ONX gurneys/ambulances in your script or package
- ✅ You may direct customers to the official ONX Tebex/CFX packages for the required asset dependencies
General asset rules: for use on licensed subscriber servers only with assets granted in your Cfx keymaster portal. No reuploading, resale, or rebranding of ONX assets (other than permitted liveries/textures where applicable). Redistribution outside your subscribed server(s) is prohibited.
❓ Frequently Asked Questions
Q: Is this a real vehicle entity or just another prop hack?
A: It is a true vehicle entity with driver and passenger seat nodes. That means it has its own physics, predictable behavior across frameworks, and seat slots that keep patients locked to the bed without giving them vehicle control or breaking on ragdoll.
Q: Do I need the ONX ambulance fleet to use this gurney?
A: No, the gurney functions standalone. However, it is engineered to pair with the ONX ambulance fleet of six models, where it can attach to the misc_s bone and align with the matching gurney mod part for a clean loader workflow.
Q: Is the push, load, and transfer scripting included?
A: No — that logic is intentionally left script-open. The package provides the asset itself so you can implement the exact behavior you want for QBCore, ESX, Qbox, or a custom framework without fighting prebuilt scripts.
Q: How do players raise or lower the gurney?
A: Holding the H key triggers the animated height adjustment. The handle also raises automatically when the gurney is in the lowered position for visual accuracy.
Q: What are the framework and game build requirements?
A: The asset is framework-agnostic and supports QBCore, ESX, Qbox, and custom setups. Game Build 3095 is required for some features. A subscription is optional, not mandatory.
🚑 Give Your EMS the Stretcher They Actually Deserve
The ONX Brute EjectoLift NX is the missing piece between an ambulance and a believable rescue. With animated height control, dual seat nodes, two gurney variants, colorable frame paint, and clean integration paths into the ONX ambulance fleet, your EMS workflows finally look as professional as your players roleplay them. Drop it on your server, wire it into your framework, and watch routine medical calls turn into the kind of scenes your community talks about for weeks.
